PARENTAL/GUARDIAN CONSENT,
RELEASE FROM LIABILITY AND INDEMNITY AGREEMENT
The Enrichment Activity Center LLC and Lets Love Music! assumes responsibility for children only during designated hours of the program. Parents/guardians are responsible for providing transportation to and from the program at the proper times.
RELEASE: I realize that participation in the programs listed on this form involves some risk but, regard the benefits to outweigh the risks. I agree to hold harmless the Enrichment Activity Center LLC and Let's Love Music!, its agents, employees, contractors, volunteers, from any and all claims sustained while participating in this program. PERMISSION IS GRANTED FOR ANY EMERGENCY MEDICAL TREATMENT NEEDED.
